 The non-profit FairCare Health System, originally piloted at the VA Healthcare Network Upstate New York at Albany, NY, is currently working with managed care organizations, hospitals and community based groups to train and implement clinical teams to deliver comprehensive end-of-life care. The clinical teams of doctors, nurses, social workers and clergy provide education, counseling, advocacy, and coordination of services, based on the FairCare Program, for those facing end-of-life situations. 
Currently, the FairCare Health System is actively working with six Upstate New York VA Medical Centers and twenty-two Community Based Outpatient Clinics, the Jefferson Health System (Philadelphia), Cigna Healthcare (Arizona), Fairview Health Services, Allina Health System and Health Partners (Minnesota), Nursing Homes, Hospices nationwide, and several self-funded labor unions.
Projects range from the implementation of fully trained FairCare
Health System teams within large managed care organizations to the creation of advanced directives programs, nursing home end-of-life programs, clergy training, EMS education, Alzheimers/FairCare, and pediatric outreach education.  The Life Institute works in collaboration with the VA Healthcare Network Upstate New York, as well as other national groups and educators to create educational tools to improve end-of-life care. The Institute has created programs to train health care workers, clergy, and community members in communicating with people facing advanced illness, as well as with their families.
